Safety is a top priority for all students, employees, and visitors. If you are experiencing an area of concern, please let us know through one of the options provided below.

Complaint / Grievance

Please use this for any complaint/grievance including but not limited to, facilities, interactions with College employees or faculty members, course grades, administrative procedures, complaints related to academic services, violations of board policies, etc.

Student of Concern Form

Please use this if you are concerned about a student’s well-being, know of a student struggling mentally, financially.

Title IX Reporting Form

This should be used to report concerns pertaining to possible violations of Clatsop Community College’s Sexual Harassment Under Title IX Policy such as gender-based discrimination, sexual assault, dating violence, domestic violence, stalking, unwelcome conduct based on sex/gender/gender identity, quid pro quo harassment, retaliation, or other forms of sexual misconduct.

Accident/Incident/Injury Reporting Form

Please use this form to report an accident, injury, illness, or property damage that has already occurred.
